The personal data I have includes data relating to your name, address, telephone number, date of birth, and any data you have given me in your original assessment relating to your health including mental health and history. I use this to enable communication to take place between us and to fulfil my professional obligations within the law. Your right to confidentiality is respected and any sensitive information will only be shared in specific circumstances. This may include further referral processes, ongoing or complimentary treatment.
I will only use your data for the purposes which it is collected and will only grant access to your data as I am obliged to by law. I will not be sharing your data with third parties unless as mentioned above for onward referral or continuity of care. This may include communication with your GP if I am sufficiently concerned about your health or safety.
Please note that the transmission of information via the internet is not secure. I cannot guarantee the security of your data transmitted to my site; any transmission is at your own risk. Once I have received your information, however I use strict procedures and security features to try to prevent unauthorised access.
As required by the The General Data Protection Regulations GDPR, my practice is registered with the Information Commissioner's Office (ICO) under the reference A8309998.
When we decide to work with each other, I will formally ask your permission for me to store your data and records according to ICO guidelines and will provide essential information for you in writing.
